Swisher Memorial Healthcare System takes pride in providing exceptional post-acute care services to those who may need rehabilitation prior to returning home. SMH Swing Bed program offers 24 hour in-hospital, skilled nursing care services plus the benefit of rehabilitation therapies in order to help patients reach an optimal level of functioning.

How Do I Qualify For Skilled Swing Bed Services?


-You must have had consecutive, 3-day, acute level care hospital stay within the past 30 days.
-You must have a need for some form of skilled nursing service or skilled rehabilitation service that can be provided in a skilled nursing facility.

Examples of skilled care include:


-IV therapy
-Wound care
-Rehabilitation therapy:
-Physical therapy
-Occupational therapy
-Speech therapy
-Respiratory therapy

How Do I Pay For Swing Bed Services?


Medicare will cover skilled swing bed if:

1. You have Medicare Part A and have days left in your benefit period available to use.

2. You have a consecutive, 3-day, acute in-patient hospitalization within the past 30 days.

3. Your doctor has determined that you need daily skilled care. Care must be given by or under the direct supervision of skilled nursing or rehabilitation staff.

Medicare coverage is limited to 100 days of skilled swing bed care. If you meet skilled criteria, Medicare will cover 100% of the first 20 days. You may be discharge before 20 days if your daily skilled criteria are met.

If you require a stay past 20 days, co-insurance dollars or self-payment will be assigned from day 21 to 100.

Non-Medicare Swing Bed services are also available. Swing Bed services covered by private insurances require a prior authorization.
